Schochet Partners with Hebrew SeniorLife to Introduce Healthy Aging Program at Fresh Pond Apartments

Building upon similar offerings at two other Schochet properties, Fresh Pond will become the first multi-family property to offer the HSL R3 program, designed to extend independent living for elderly and disabled affordable housing residents

CAMBRIDGE, MA, JULY 28, 2025 -- Schochet and Hebrew SeniorLife (HSL) this month introduced the nationally recognized R3 Program (Right Care, Right Place, Right Time) at Schochet’s Fresh Pond Apartments, a 504-unit affordable housing complex in Cambridge, MA, marking the first time the program has been made available at a multi-family property and offering the 230 older adults at Fresh Pond the resources to live healthy, active, vibrant lives.

Working with the on-site Schochet resident services team at Fresh Pond, the R3 program will offer place-based services to improve older adults’ emotional and physical well-being, increase independence and longevity, and reduce health care-costs. Hebrew SeniorLife will offer an on-site nurse, blood pressure clinics, health and wellness education, exercise classes, and assistance with benefits and access to resources.

This is the third Schochet-managed property and first multi-family housing property to offer R3, an innovative program that integrates wellness teams into senior housing communities. Others include Riverside Towers, a joint venture with Jonathan Rose Companies  in Medford, MA, and Retirement Housing Foundation’s Hamilton Wade/Douglas House in Brockton, MA.

R3 was created by Hebrew SeniorLife to support residents in proactively managing their health by effectively linking housing and health care. The goal of R3 is to improve lives, reduce costs, and help older adults stay independent and at home longer. The program won the Pioneer Institute 2017 Better Government Competition,  The John A. Hartford Foundation 2021 Business Innovation Award, and the 2025 Association of Jewish Aging Services Innovation Igniter Award. The R3 model includes a Nurse Care Manager, Wellness Coordinator, and Fitness Specialist working on-site with the existing housing team to provide supportive services to residents.

Staff reach out regularly to individual residents to engage them around their health and wellness. Through these conversations, they work to understand the barriers residents might face in remaining healthy, then follow up to address needs and connect them to resources. The wellness team also provides individualized case management and support, all centered around the resident’s choice and preferences. The Fresh Pond Healthy Aging Team Members will help residents aged 62 and older with accessing and coordinating these services.

In addition to the work of the wellness teams, all staff in the community - including maintenance, administrators, and social work - are trained to contribute to care coordination efforts.
